Although weight loss is routinely recommended for obese person, the observational literature is controversial in respect of prolongation of life longevity after body weight loss. The positive long-term outcome has been found only in intentional weight loss in obese people with obesity-related comorbidities. The main task in obesity treatment strategy is "primum non nocere". The authors analysed the short-term effect and acceptability of low calorie diet (5 MJ) on health state of 150 obese people (BMI x = 35.9 kg/m2).

In the first part Freud's clinical theories and models and the ego psychological theory were described from which ensues a general pattern of objective relations. The second part of the paper attempts a synthesis of the most important psychoanalytical theories, the creation of a uniform model of psychic functioning using an epigenetic pattern. The author analyzes in more detail developmental trends of these mental functions which are considered in psychoanalysis decisive for adaptation.

Altruistic behaviour belongs to so-called prosocial activities. It is focused on the benefit of another human being who is in want or is threatened. Its substantial features include voluntariness, unselfishness and specific motivation, based on human mutuality and unity. Altruism was at the very beginnings of organized health care. As a result of the development of medicine the human attitude of the doctor to the patient, and thus also psychological care and treatment were pushed into the background.

In this country, the situation in the field of care for people suffering with acute psychic changes following psychotraumas and other changes which we generally describe as a crisis in one's life was unsatisfactory for a long time. The cause must be sought not only in automatization of medicine, inadequate professional training and lack of altruism on the part of doctors.
