It is predicted that more Australians will retain their natural dentitions into old age and their dental needs will have to be met with a wide range of dental services by dentists acting with understanding, kindness, and sympathy. The empathy to do so requires experience of appropriate delivery systems in the environment in which the elderly live. For them the ability to eat a balanced and varied diet and to manage personal oral hygiene is very important for their well being. Undergraduate education must be adapted to strengthen the social orientation demanded.

If it is not a naÔve expectation for dentists who have been beneficiaries of public generosity to share their good fortune with the public that made it possible, there may be a rational basis for enhancing the role of dental education in improving access to oral health care by promoting-but not requiring-a voluntary service commitment after graduation commensurate with the magnitude of the subsidy received.

The most important mission of dental education is development of student professionalism. It is only within the context of professionalism that specialized knowledge and technical expertise find meaning. Altruism, integrity, caring, community focus, and commitment to excellence are attributes of professionalism. Its backbone is the obligation of service to people before service to self--a social contract. Professionalism can and should be acquired by targeted interventions, not as an assumed by-product of dental education.

Leaders of health professional schools often support community-based education as a means of promoting emerging practitioners' awareness of health disparities and commitment to serving the poor. Yet, most programs do not teach about the causes of health disparities, raising questions regarding what social and political lessons students learn from these experiences. This article examines the ways in which community-based clinical education programs help shape the subjectivities of new dentists as ethical clinician-citizens within the US commodified health care system.
